Consent and Intake Forms


Family Therapy, Child Therapy and Parent Coaching

You will receive a Consent Form and an Intake Form to complete before your session. If your family is involved in a co-parenting situation, you will also receive a Families in Transition Waiver to complete

If you are a parent receiving the Intake Form, even if you are interested in child therapy, we recommend that you complete this form from your own perspective, providing your associate with background information about your family to inform your initial session. Please feel free to include any information or details about your child specifically as well that you feel would be important for the associate to know about. 

We ask that you kindly complete the Consent Form (and Families in Transition Waiver, if applicable), prior to your session with the associate. These consents are mandatory as we require consent prior to beginning any therapeutic work. If the consent form(s) is not completed in advance, your associate can assist you in filling this out during your session time.

Occupational Therapy

In-Office Occupational Therapy Sessions

If you have scheduled an in-office Occupational Therapy session, you will receive a Consent Form and a combined Occupational Therapy Consent & Intake Form. If your family is involved in a co-parenting situation, you will also receive a Families in Transition Waiver to complete

Psychological Assessments

You will receive a Consent Form and a Psychological Assessment Consent Form to complete prior to your session. If your family is involved in a co-parenting situation, you will also receive a Families in Transition Waiver to complete.

If you are completing a Psychodiagnostic Consultation session with our Psychologist, please note that you will not receive the Psychological Assessment Consent Form, but an Intake Form.
